The Social Services Department works closely with tenants and families in placement admission to the facility.  A social worker and registered nurse meet with the tenant and family prior to admission to complete an assessment which assists in planning for the individual's needs.  The main goal of the social worker is promoting tenant advocacy.  The social workers promote the tenants' independence and make sure the tenants' psychosocial needs are met within the facility.  The social service staff meet with each tenant after admission to discuss goals and will conduct care conferences as needed. 

 

At MeadowWood Assisted Living we promote the tenants' right to be treated with dignity and respect, as well as the need to feel cared for, secure, welcome and appreciated.  Social Services also assists tenants and families with discharge planning to help make a smooth transition to another facility or to reture home.
